This is the original questionnaire that was used for this Bar Guide. Now we request that those wishing to add their personal input to this guide go to the bars they are interested in commenting on and writing comments there. You can still send in this questionnaire, but direct input is preferred.

This questionnaire is for the 2010 Official Puerto Vallarta Bar Guide. Here it is possible to voice your opinion on the bar scene in this town. The results are being tabulated and published here at PVScene.com periodically.

The Rules are Simple:

• One form per person per bar.
• If you are sober enough to figure out this form, you are sober enough to evaluate bars.
• Add reviews of any bars that you think we have missed (as long as they are in Puerto Vallarta.

Of course, personal information gathered from this form (your name and email) is totally private and not shared with anyone.
